Yasmeh GI ("the practice") respects your privacy. This policy describes what information this website collects and how it is used.

Information you provide

If you submit the consultation request form, we receive the contact details you enter (name, phone, email, preferred contact method, and general reason for your inquiry). This information is transmitted via our form-delivery provider to the practice's email and is used solely to respond to your inquiry. Please do not submit medical details through the website; a secure method will be provided when appropriate.

Cookies and tracking

This website sets no cookies of its own. It uses no advertising trackers, no analytics, and no social media pixels. Because nothing here tracks you, there is no cookie banner to accept or decline. Our hosting provider (Microsoft Azure) may log standard technical information such as IP addresses for security and operational purposes.

Outside services this website relies on

A small number of outside services make this website work, and each receives only what it needs:

• Microsoft Azure hosts the site and may log technical request data for security.
• Google Fonts serves the typefaces on these pages. Your browser requests those font files from Google, so Google receives your IP address as part of that request.
• FormSubmit delivers contact form submissions to the practice email and uses them for nothing else.
• Zocdoc operates the optional booking link on our contact page. If you book through it you leave this website, and Zocdoc's own privacy policy governs what happens there.

Text messaging (SMS)

If you check the text message consent box on our contact form, you agree to receive SMS text messages from Yasmeh GI at the phone number you provide. We collect your phone number and a record of your consent for this purpose, and we use them only to schedule appointments, send appointment reminders, and respond to inquiries you send us.

SMS consent is not shared with third parties or affiliates. Your phone number and your SMS consent are used only by the practice and by the messaging provider that delivers those messages on our behalf, and they are never sold, rented, or shared for marketing purposes.

Consent is optional and is not a condition of receiving care. Message frequency may vary, and message and data rates may apply. You may opt out at any time by replying STOP to any message, and you may reply HELP for assistance. Full details are in our SMS Terms of Service.

Text messaging is not a secure channel. Please do not send symptoms, diagnoses, test results, or other medical details by text; the office will provide a secure method when clinical information needs to be exchanged.

How information is shared

We do not sell or rent your information. Contact-form submissions pass through our form-delivery provider solely for delivery to the practice. We may disclose information if required by law.

Health information and HIPAA

This policy covers information collected through this website only. Medical records and other protected health information created once you become a patient are governed by HIPAA and by the practice's Notice of Privacy Practices, which you receive at your first visit. Anything you send through the contact form before you are a patient is an inquiry, not a medical record.

Children's privacy

This website is intended for adults and is not directed to children. We do not knowingly collect personal information from anyone under 18 through this site. If you believe a child has submitted information to us, email info@yasmehgi.com and we will delete it. When a minor is a patient, their information is handled through the practice's clinical records rather than through this website.

Your choices and your rights

You may email info@yasmehgi.com at any time to:

• Ask what contact information we hold from your website inquiries.
• Ask us to correct it.
• Ask us to delete it.
• Withdraw your consent to text messages, which you can also do by replying STOP to any message.

We will respond within 45 days. If you are a California resident you may also have rights under the California Consumer Privacy Act, and we honor the requests above regardless of whether that law applies to the practice. You will never be denied care or treated differently for exercising any of these choices.
